DO I NEED A DOCTOR’S REFERRAL?
In Ontario, you do not require a Doctor's referral. However, if you have extended insurance, they may require a Doctor's referral, in order to reimburse you for Physiotherapy Services. If you are unsure, please call your insurance company to confirm.
WHAT IS THE INTAKE PROCESS LIKE?
Once you get in touch with Zev Herskovits PT, a Welcome Email will be sent to you, and you will be able to create a profile and complete the intake form via any device. If you are unable to complete the profile or intake forms, this process will be completed at your initial visit.
WHAT IS THE COST ASSOCIATED WITH YOUR SERVICES?
Initial Assessments are $150 and Treatment sessions are $125. Appointments in Etobicoke, Mississauga and Scarborough are $175 for Initial Assessment and $160 for Treatment sessions.
HOW LONG ARE THE APPOINTMENTS?
Initial Assessments are scheduled for 45 minutes, and Treatment sessions are scheduled for 45 minutes or 30 minutes.
WHAT TYPES OF CONDITIONS DO YOU TREAT?
Sports related injuries (sprain, strains, tendinitis, tendinopathy)
Post-op rehabilitation (ACL, Rotator cuff repairs, joint replacements, Lumbar and cervical fusion, Labral repairs etc...)
Acute and Chronic injuries
Repetitive strain injuries
Neurological conditions (MS, Stroke etc...)
Orthopedic conditions (Fractures, non-surgical rehab etc...)
Headaches
Chronic pain
Geriatrics (deconditioning, Palliative Care).
Hospital care - Intensive Care Units and Medical/Surgical Units (varies by hospital)
